Year 1 of the LHMG, which began in February 2014, served as a planning period, focused on the formation of the local steering committees made up of Miami Gardens community members and stakeholders, selection of Health Impact Areas (HIAs), and development of a Community Action Plan. The mission of LHMG will be achieved through the implementation of the Community Action Plan developed during this period. Developed by the Host Agency, and based on the Health Impact Areas (HIAs) areas selected by the Host Council, the Community Action Plan provides a roadmap for the assessment, planning, capacity building and implementation of strategies designed to produce fundamental changes in services, policy, systems and environments within the community to materially improve the long-term health and quality of life of residents. These impact areas are Healthy Eating and Improved Nutrition; Mental Health Services; Primary Health Services; Physical Activity; and Alcohol, Tobacco and Other Drugs (ATOD).
The HIAs areas were selected after a vigorous data gathering and analyzing process, including community planning and needs identification; looking at key trends that influences the health of residents in Miami Gardens; and conducting a community Check In process, where resident input was solicited through a Community Survey and Focus Groups. The process furthered the mission of the LMHG by reaching deep into the communities we serve to better understand and address the issues of those in need.
